Primary Health Organisations known as PHO’s provide health services either directly or through it’s contracted providers. The aim is to provide and maintain the health of the enrolled PHO population, by ensuring that general practice services are connected with other health services to ensure a seamless continuum of care.
The Organisation
Kimi Hauora Wairau Marlborough Primary Health (MPH) promotes and supports community health services by leading and coordinating a range of healthcare services to an enrolled population of over 46,000 people. It achieves this by contracting services to General Practices, Marlborough Urgent Care, and other Primary Health providers such as Māori and Pacific Providers.
The Job
To provide advanced clinical expertise and leadership that promotes health, prevents disease and to diagnose, assess and manage people health needs.
This is provided within an interdisciplinary model that best meets the patient requirements and is in line with the vision and values of Kimi Hauora Marlborough Primary Health Organisation (KHW MPHO).
The Ideal Candidate
- Clinically focused master's degree in nursing
- Working towards or is Registered as a Nurse Practitioner with NZNC
- Current practicing certificate
- A minimum of five years of recent clinical experience in the Health of Older Person nursing
- Functioning at Expert level on the Senior Clinical Career Pathway
- Knowledge of current issues within nursing and in Health of Older Person nursing
- Sound knowledge and understanding of medical/legal and ethical responsibilities
